After booking an initial assessment either through Mindbody or with our administrative staff, please complete the forms below. The links will redirect you to a secure page where you can fill in your personal information. Once completed, either email them to info@burlingtonphysiotherapyhc.ca or bring them to your first appointment. If you would like to complete these forms in person instead, please arrive 15 minutes prior to your appointment.
